AUTHORIZATION TO ENTER INTO A PROFESSIONAL SERVICES AGREEMENT WITH AGINGNEXT, INC. FOR PROFESSIONAL CASE MANAGEMENT SERVICES (FUNDING SOURCES: CDBG FUND AND GENERAL FUND)

Body

SUMMARY

The contract-based Senior Case Management Program provides senior residents of Claremont with comprehensive resources for social services. The program operates out of the Joslyn Senior Center Monday through Thursday from 9:00 AM to 2:00 PM, and serves as a safe place where participants and their families can obtain sound, impartial advice and get connected to additional resources that may benefit them.

Since 2010, AgingNext has been the sole provider of these services for the City of Claremont. Throughout this long-term collaboration, AgingNext has demonstrated proven competence in providing senior social services, in-depth knowledge of local resources, and the ability to handle crisis situations.

RECOMMENDATION

Recommended Action

Staff recommends the City Council authorize the City Manager to enter into an agreement with AgingNext, Inc. for professional case management services for a term of one year in an amount not to exceed $35,120, with the option to extend the agreement on an annual basis for four additional one-year terms until June 30, 2029. Additionally, if the agreement is extended, the compensation will be increased at an amount commensurate with the April-April Consumer Price Index (CPI), not to exceed three percent each year.

FINANCIAL REVIEW

AgingNext will provide the City with twenty hours of case management and other social services per week onsite at the Joslyn Senior Center, the Blaisdell Community Center, and at other facilities and events as needed, at an annual cost of $35,120. Should the agreement be extended, the compensation will be increased at an amount commensurate with the April-April CPI each year, not to exceed a 3 percent increase each year. The total cost to operate this program for five years would not exceed $186,458.

The staff cost to prepare this report and administer this program is estimated at $741 and is included in the operating budget of the Recreation and Human Services Department.

ANALYSIS

Senior case management services are offered at the Joslyn Senior Center Monday through Thursday from 9:00 AM to 2:00 PM. Case management services are also offered monthly at the Blaisdell Community Center, at community events, and through in-home visits as needed. While information and referrals are available to any seniors who call or visit our centers, the more extensive case management services are reserved for Claremont residents only.

Examples of typical services include information and referral to local service agencies and community-based organizations, food pantries, support groups, and affordable housing options; emergency fund requests; help for older adults and their families with navigating care services and healthcare networks; and case management for clients needing ongoing support or follow-up with working through difficult situations. Typically, the program serves approximately 75 older adults with various levels of ongoing case management services and addresses over 1,400 resource and referral contacts each year. Additionally, the program maintains the Phone Assurance Line (PAL), an outreach program to ensure that older adults needing social connection receive a friendly daily check-in. The case manager also coordinates initial intake for the Safely Alert List, a City program that ensures that homebound seniors or those with special needs can be located and assisted in the event of a major emergency.

Partnership Benefits

AgingNext, which serves the Inland Empire, Pomona, and East San Gabriel Valley, is the primary provider of many senior services programs within the Claremont community. Examples include the Memory Care Center based out of Larkin Park, the Care Partner Program, and Resource Specialists, a family caregiver support program. This agency is also a regular partner of many community events (e.g. health and wellness fairs, scam prevention events, and more). These programs directly complement the City’s programs and services offered at the Joslyn and Blaisdell Centers. As such, the partnership will continue to promote the accessibility of services to the Claremont community. This partnership saves the Senior Program staff countless hours by redirecting clients to a more qualified provider, allowing staff to focus on City-related work plan items, as well as better serving clients onsite.
