Title

AUTHORIZATION TO AMEND THE AGREEMENT WITH AGINGNEXT TO EXTEND THE TERM AND ADD COMPENSATION FOR SENIOR CASE MANAGEMENT SERVICES (FUNDING SOURCES: COMMUNITY DEVELOPMENT BLOCK GRANT FUND AND GENERAL FUND)

Body

SUMMARY

The contract-based Senior Case Management Program provides senior residents of Claremont with comprehensive resources for social services. The program operates out of the Joslyn Senior Center and serves as a safe place where participants and their families can obtain sound, impartial advice and get connected to additional resources that may benefit them.

AgingNext has been the sole provider of these services since 2010. Throughout this long-term collaboration, AgingNext has demonstrated proven competence in providing senior social services, in-depth knowledge of local resources, and the ability to handle crisis situations.

In preparation for the upcoming contract term, AgingNext conducted a review of the operational costs associated with maintaining the assigned Case Manager position. AgingNext has indicated that the current contract amount no longer fully reflects the cost of sustaining the existing service level. As a result, AgingNext submitted a proposal requesting an increased annual contract amount.

RECOMMENDATION

Recommended Action

Staff recommends the City Council authorize the City Manager to amend the existing agreement with AgingNext to extend the term through June 30, 2027 with the option to extend for two additional one-year terms, increase compensation to $38,500 per year, and authorize an annual increase in an amount equal to the April-April CPI, not to exceed 3 percent each year, not to exceed $119,000 over the three year term.

ALTERNATIVE TO RECOMMENDATION

In addition to the recommendation, there is the following alternative:

•                     Request additional information from staff.

FINANCIAL REVIEW

AgingNext will provide the City with twenty hours of case management and other social services per week onsite at the Joslyn Senior Center, the Blaisdell Community Center, and at other facilities and events as needed, at an annual cost of $38,500. Should the option to extend the agreement be exercised, the compensation will be increased at an amount equal to the April-April CPI each year, not to exceed a 3 percent increase each year. The total cost to operate this program for three years would not exceed approximately $119,000, inclusive of annual CPI adjustments.

The staff cost to prepare this report and administer this contract is estimated at $717 and is included in the operating budget of the Recreation and Human Services Department.

ANALYSIS

Current Usage

Senior case management services are offered at the Joslyn Senior Center Monday through Thursday, 9:00 a.m. to 2:00 p.m. Case management services are also offered monthly at the Blaisdell Community Center and through in-home visits as needed. While information and referrals are available to any seniors who call or visit our centers, the more extensive case management services are reserved for Claremont residents only. AgingNext meets all the responsibilities and qualifications detailed in the Professional Services Agreement.

Services provided include individual case management; information and referral services, including connection to community-based organizations, food pantries and support groups; emergency assistance application support; resource assistance for housing and benefits programs; and help for older adults and their families with navigating care services and healthcare networks. Additionally, the program maintains the Phone Assurance Line (PAL), an outreach program to ensure that older adults needing social connection receive friendly, frequent check-ins. The case managers also coordinate initial intake for the Safety Alert List, a City program that ensures that homebound seniors or those with special needs can be located and assisted in the event of a major emergency. Typically, the program assists with approximately 1,700 information and referral contacts each year, along with providing assistance with 400+ cases requiring more in-depth, ongoing guidance.

Partnership Benefits

AgingNext, which serves the Inland Empire, Pomona, and East San Gabriel Valley, is the primary provider of many senior services programs within the Claremont community. Examples include the Enrichment Center for adults living with memory impairment, the Senior Companion Program, and transportation assistance programs. This agency is also a regular partner of community events (i.e. health and wellness fairs, scam prevention events, and more). These programs directly complement the City’s programs and services offered at the Joslyn and Blaisdell Centers. As such, the partnership will continue to promote the accessibility of services to the Claremont community. This partnership saves the Senior Program staff countless hours by redirecting clients to a more qualified provider, allowing staff to focus on City-related work plan items, as well as better serve clients onsite.
